Participant Support:

• Deliver high-quality, person-centered care aligned with program standards and regulatory requirements.

• Assist with personal care tasks such as bathing, dressing, feeding, and hygiene.

• Support mobility needs through safe lifting, transfers, and repositioning.

• Foster social interaction and community involvement through daily activities and outings.

• Model positive behavior and encourage life skill development.

• Communicate using verbal and non-verbal techniques tailored to participants’ needs.

• Respond appropriately to behavioral challenges using respectful intervention techniques.

• Administer and document medications in accordance with policies.

• Transport participants to appointments, errands, and recreational activities, including out-of-town travel.

Program Duties:

• Support implementation of individualized plans in collaboration with participants, families, and other resources.

• Maintain a clean, safe, and home-like environment.

• Prepare meals based on dietary needs and preferences.

• Use and maintain household and agency equipment (e.g., kitchen appliances, lawn tools, vehicles).

• Provide on-the-job training to new staff as assigned.

Administrative Duties:

• Maintain accurate documentation of care, finances, and inventory.

• Provide written and verbal reports as needed.

• Attend meetings and training sessions.

• Monitor and report on safety and facility issues.

Day Program Specific:

• Assist in planning and leading community outings and career development activities.

• Track progress and explore employment or volunteer opportunities for participants.

• Manage supplies and resources for daily programming needs.

• Accompany participants to medical appointments when required.

Residential Program Specific:

• Support daily living activities like laundry, housekeeping, and budgeting.

• Help create a warm, supportive, and engaging home environment.

• Plan and prepare balanced meals.

• Monitor home maintenance needs and report concerns.
